Squonk, a nice little trick that I used on my thumbpick (standard Jim Dunlop) to sort out the blood supply issue is to drop the pick in a cup of hot/boiling water. It becomes very easy to bend the part that goes over the thumb. I opened it up a bit, stuck it on my thumb (when it'd cooled down slightly) and stuck my thumb under the cold water tap to harden it again. It worked like a charm! Much more comfortable now!
Something else I picked up on the net is to file the tip of the pick down. I cut the tip of the thumbpick back until it protruded about the same amount that I flat pick would. That made all the difference in making the thing useable!
