I got one of the Dimarzio superswitches (EP1112).
Yeah - those are the Oak Grigsbys. I just wanted to make sure you didn't have the Ibanez Super or a standard CRL 5-way.
1. Red wires from pickups go to left (as pictured) lugs on pots.
2. Pot wipers (Centre lugs) go to the switch (where the red wires go in the diagram)
3. Bottom two lugs on the switch are the output (connected together and with a faint white wire to the pot in the diagram), these stay connected and go straight to the socket.

2.) I'm guessing that the outputs from the pots are the grey wires in the diagram?
Yes on the volume, no on the tone. As a volume, wiper (centre) lugs will be outputs.
3.) What is the black wire going from the volume pot to the switch and from the volume pot to the tone pot in the diagram? Do I need those there too?
Earth connections. Yes, you need them - used as part of the coil-cutting circuit .
4.) Which posts are the switch outputs and are the white and black wires of the pups wired as per the diagram?
In this case (switches can go both ways), the two lugs right at the bottom.
