It says Electar Epiphonic Six - assuming it was made 1998 with the guitar.
How old it is, how the quality is, etc

Usually found in Epiphones. Preamp designed by Shadow, Germany. Not bad, but the pickups they were used with were usually ceramic piezos, so they are often a bit bright and percussive.
