This is gonna sound strange but at one point a few people I knew were using the triangular plastic binders used to hold documents together, they would cut them shorter and attach them to the underside of their headstocks with double sided tape. This only really works for 6 in line style headstocks and only if you use a relatively big plectrum, but it is a pretty nifty alternative and it looks cool when you reach around to your headstock to grab a plectrum (if you kinda over-emphasize the movement in a Steve Vai-ish kinda way).
Also Ibanez make a double sided tape style microporous thing that is designed to stick under your headstock in much the same way, this is no doubt the one Steve Vai actually uses.
